visa documents for usa

How can I ensure a multiple entry USA visa (B2)? And what documents should I give to show “purpose of my trip”?
I am a university student in Canada (citizen of India) who plans to visit the states to travel. I must obtain a B2 visa to enter the States. My questions are:
1. If I want to be able to visit California (in June) as well as Florida in (August) do I need to apply for a visa twice? How would I go about obtaining a multiple entry visa?
2. Part of the requirements for a B2 visa is to provide “Evidence which show the purpose of trip”. Do I have to book plane tickets, or a hostel reservation? (I won’t be visiting anyone in the States)
What should I do?
All B-2 visas are multiple entry. B-2 visas are for the purpose of tourism , so you just need to say you are going as a tourist…name places you intend to visit…cities, tourist attractions. You do not have to have reservations or tickets first.
